In case anyone is scrambling to find a MD to process the medical exam before the Aug. 17 dealine, I would like to recommend
Dr. Jin Kwan Kim

I chose his office from the list of docs that my attorney provided me.
He charged $105, which was considerably lower than other places I've called.
But who knows if he decides to charge more if there is demand for it.
The lab (for HIV and syphillis) was just below his office which was convenient.
His office had the report ready in the sealed envelop on the 3rd day when I went back to get the TB test read since the result was negative and didn't have to get a chest x-ray.
I also had proof for other vaccinations. They offer vaccinations at extra costs, but they were not terribly expensive. If you have health care coverage and know what vaccins you need, you can get them at your regular Dr's office and take the record to this office.
FYI, if you don't have records for vaccinations, it would be best to get them and get it into the records, but be aware of potential physical response if you get more than 1 vaccine.
My brother got 3 vaccinations in one day and had fevers for 3-4 days. We almost took him to the emergency room.
