Including name in Birth certificate: Urgent

Dear All,

My birth certificate do not have my name. Can you please tell me how to include my name in my birth certificate in Kolkata. Please tell me what are the documents my parents need to show to get my name entered in the birth certificate. Kolkata municipal corporation is messy so I would appreciate if anybody with similar experience can share detail information. I dont want to make my parents go there more than once, so would probably want to give then as much detail documents as possible.

Your parents need to get a lawyer, sign an affidavit that includes a photocopy of your current BC, take that proof of your birth, signed by the lawyer and your parents: to the Kolkata Municipal Corporation. They will issue a new BC with your name on it.
