Hi,

I was born in India and my name on my birth certificate contains initials :(.

Since my mom and relative who were there at the time of my birth are visiting me in the US now, I would like to get an affidavit from them stating that the birth certificate is infact mine and that name is my true name.

Can you please advise on what procedure I should follow here in the US to get an affidavit like that? I would really appreciate a detailed response. Thank you.

I do not know if for your case (change in name) a simple US notary will do but my wife did not have BC. We got a Non availability certificate from Municipality and Notary by my in-laws where they were in US. It is simply a notarized affidavit. My attorney says this will work

I don't think birth certificates issued by the Indian Embassy are accepted by USCIS for GC purpose. They are issued only on the basis of Indian passports. Correct me if I am wrong.
