Hi All,
I am going to apply for G639 to get my I140 approval/application copy. I have few questions, please help
1. Is notary required for g639 or I can select "Sworn declaration under penalty of perjury". If I select latter do I need to fill in any form/doc? This is Section 4 "Verification of Identity"
2. Is the G639 form from USCIS enough or is a covering letter required along with it?
3. In section 2 "Description of Records requestion" it asks for I-94. Should this I-94 be the lastest one (extension one) or the one which was issued when I entered US?
4. I want only my I-140 details, so should i select "All of my records" or "A portion of my records" in Section 3 "Subject of record consent to release information"
