I am starting this thread after I experienced that CIS Ombudsman is actively helping EB as well as other immigrant community in getting their problems with USCIS resolved very quickly. We are fortunate to have such a platform to log our problems and get solution.
I am sure many of us have received similar help from the office of CIS Ombudman. Please thank them.

How and when are we supposed to contact Ombudsman for help?
Are we supposed to wait first for some stipulated time or we can contact when our PD is current?

Ombudsman helps in certain limited circumstances, particularly when an adverse decision by USCIS puts you in emergency situation. Following link should help determine when to contact them:-

Our PD : Dec 22nd 2003, EB2 India. It was pre-adjudicated on Sept 18th 2008.
My petition was approved 4 months back, after calling 100 times USCIS.
In my wife's case we spoke with IO on Sept 4th.
Created Service Request on 5th Sept.
During September 2nd week spoke to IO, IO sent an email to IO who is looking at petition.
During September last week we got Service Request generic response "Wait 180 days" which I have seen 2 times in my case .
On Sept 29th sent 7001 form to Ombudsman. Ombudsman office might have received the application on or before 1st October.
October 2nd 2009 my wife's petition approved.
So if your case is genuine case problem. Don't waste your time calling Immigration officers. Send 7001 form to Ombudsman's office.

I just noticed on IV homepage that ex USCIS ombudsman is now IV board of advisors.

Take an infopass. They can tell if your case is pre-adjudicated or not.

I have taken infopass 2 times in last 2 years, and they just tell me - we cannot tell anything about that info from the "screen" they have access to.
They just give a vague answer that everything is in order but your visa numbers are not available for your dates, you need to wait for visa number

They just give a vague answer that everything is in order but your visa numbers are not available for your dates, you need to wait for visa number
I went twice, both the time they gave me that info. Not only that, they can tell you if your case was assigned to an IO or not and also when the IO reviwed you application lately. You will have to specifically ask them.

Briefly Explain about case PD, Received Date, type of Case. Explain current Priority date, out of process date etc.. whatever suits your case.
After that explain about creating Service Requests, Infopass appointments didn't make any progress.
Google 7001 you can find lot of Descriptions in site for writing Description.
Could you please tell me the reason you mentioed for "Description of your problem".
