PIO form question

I am applying for a PIO card for my baby (US citizen by birth). Anyone who has filled in the PIO form please help.

Is step 13 optional? I believe if we already have given the details in step 12 do we still need to fill in step 13? If so, what do we provide for proof of residence?

12) Whether the applicant or either of his/her parents or grand parents or great grand parents held an Indian passport
at any time, if so details thereof: (enclose photocopy of relevant documents)
