Please check with Indian consulate/embassy jurisdiction, where ever you reside. On the Indian Consular services website, the birth certificate issuance details are available. Please review the link below which mentions about issuance of birth certificate.

I do not know if for your case (change in name) a simple US notary will do but my wife did not have BC. We got a Non availability certificate from Municipality and Notary by my in-laws where they were in US. It is simply a notarized affidavit. My attorney says this will work

I don't think birth certificates issued by the Indian Embassy are accepted by USCIS for GC purpose. They are issued only on the basis of Indian passports. Correct me if I am wrong.
