FOIA Request letter for I-140 form

Hi,
I had an I-140 approval form with my old company, now I moved to a new company and applying for GC process. For porting, my company is asking an approval copy of the previous approved 140 which i do not have and so my new company recommended me to request the form from FOIA (freedom of information act). Does anyone now how to request a letter with them. If you can help me in providing a sample format it would be really great.
