Options to go back to USA ?

Hi,
I used to work in USA for company A, and they were processing my GC.
I got my I-140 approved and based on that the H1 extn too was approved by USCIS in September 2011.
Also, on 17th September 2011, my 6 years was ending on H1.
I knew that since I have H1 extension for another 3 years, I can stay in USA.
However, due to some personal reasons I moved back to India in October 2011.
I resigned from Company A, and joined another company B in India.

its July 2012 now, and I wish to be able to go back to USA.

Question#1 - Can I apply for new H1 as per the USCIS process and if yes, when at the earliest can I apply ?
Question#2 - Would it be ok to use the approved H1 (the latest extension) to try for a transfer to the new employer, so that I dont have to go through quota mathematics ?
Question#3 - Can I do anything with the approved I-140 ? I have no intentions of joining the old employer A again.

Most important question - what options do I have to be able to go to USA on an H1-B visa ?
