How to get a copy of certified ETA750?

Dear friends,

Does any body know how to get a copy of certified ETA 750? My boss and I both received copy of Final Determination letter. Although it says "Form ETA 750 has been certified and is enclosed", there's nowhere to find it. My attorney hasn't received anything yet. It was approved on July 18. I tried to call Dallas BEC at (214)2379111, the lady said "once the ETA 750 is certified, it's out of our hand. The only way to get it is thru USCIS.gov web site". And she doesn't even know the exact link on USCIS.gov web site where I can obtain a copy.
I've been waiting for two years for this RIR LC and now I can't file I-140 because of this missing certified ETA 750.
What should I do? Any suggestion is welcome!

Try this link. It is about the 9089, but maybe works for 750 also

It is of no use to any member as they will only accept requests from the USCIS

Requests for a duplicate ETA-750 must be initiated by USCIS. The following steps are suggested when requesting a duplicate ETA-750 through USCIS. Please include on the top of the I-140, Petition for Alien Immigrant Worker, a cover sheet (preferably highlighted with colored paper) stating the following:
