PLEASE HELP: Passport renewal rejected at San Francisco consulate

Hi,
I applied for my passport renewal at the CGI San Francisco.

But the application and all the supporting documents were returned back with the message: " Wrong Application Form. Please use passport form for Consulate General of San Francisco".
The only application form I could find was the Online Application form in the vfs website.

The application form I had used was the Online Application Form present in VFS website. Please find it attached. The link used for this was https://passport.gov.in/nri/OnlineRe...sp?pocode=USAS. I navigated to the link from the VFS website page: India Passport and Consular Services Information - USA - Online Application Form.

I also submitted all supporting documents (Performa Verification Form, Notarized passport/visa/proff-of-residence, MO, etc.) with pictures.

Has anyone faced this issue - should I just re-send assuming that it was a VFS goof-up?

Kindly help as my H1 expiration is nearing soon and I need the passport renewed quickly.

Did you mail it to the following address?

In the first link to passport application, you will have to fill up everything online and then press print and save at the bottom. On the next page it will ask you to select an appointment date, do so sometime in the next month and them proceed to print the application form. DO not worry about the appoint date. Just mail the application with notarized copy of your H1B approval, notarized copy of the passport (first 3 and last 2 pages), notarized copy of the visa, notarized copy of the I-94. Attach a money order + $20 for shipping. YOu should be good to go.

Yes, I did (but in New York).
There are variety of Renewal Application forms available online and i downloaded one that looked clean (guess it is from ) used it for submission. COnsulated rejected that saying it is not a valid one.

Please use the application form on the SFO Embassy site. You may have to fill partial application online, print it with code, and hand fill it and then submit such application.

I followed the recommendations made above and got the renewed passport
