Immuzation record with no date stamp

Hello,

I became current in the Feb bulletin.I have my immunization records from back home in India but the doctor's notes do not have a date stamp,it has all other details like his registration,phone#,etc.

Will the civil surgeon accept this without the date stamp.

Any info is appreciated.

Better just call the surgeon whom you are going to visit and ask him if he accepts this or not.

MMR it may not matter because once you get it you don't need to get it again. For others date is important because if its not within last 10 years you have to get it again anyway.

Does the record state when you got your vaccines? if yes, just take your indian immu record to your PCP and ask his nurse to fill it out on their standard form. Tell them you need it for immigration medical. They should be able to take care of it for you.

Thanks for the replies,I called the civil surgeon,they were ok..
