Here is my story:
I took all my medical exam on Feb 2012..
All my medical exam's are normal.. but my Doctor forgot to check the section C " No Class A/B condition"... I was really worried ....I took the sealed envelope to my Doctor & they were simply saying its our over look.. I don't know what to say.. but they corrected the form & i took the recommended MMR vaccination.. I returned the sealed envelope to attorney to respond to USCIS..
if any need more infor... please let me know.. I went thru very hard.. hope some one will get benefit this..
RFE details:
1. Your adjustment application must be accompanied by documentation establishing your identity, therefore, submit a clear photocopy of a current photo identity document showing your picture, name and date of birth. Acceptable documents include passports, drivers license, state of national identification cards or other identification issues by a government agency
The photocopy must clearly show your facial features & biographical information. Color photocopies slightly enlarged from the original provide the best resolution.
Note: Consulate-issues passports are acceptable identification documents.
2. The medical examination report, Form I-693, submitted with your AOS doesn’t contain any result in Section C. Other Class A/Class B conditions for Communicable Disreses of Public Health Significance. Your original Form I-693 is hereby returned to you in the enclosed sealed envelope. Take this letter and your enclosed Form I-693 to the civil surgeon who conducted your medical examination. ONLY THE CIVIL SURGEON SHOULD OPEN THE SEALED ENVELOPE. The civil surgeon must complete the omitted potions of the Form I-693
Important warning: Only a designate civil surgeon authorized by USCIS to conduct medical examinations is qualified to complete the FORM I-693. The civil surgeon must sign and date the Form I-693. The Signature by a nurse or physician’s assistant are not acceptable. You must also sign the form I-693 when instructed to do so by the civil surgeon. When appropriately completed & signed the ORIGINAL Form I-693 must be sealed in separate envelope by the civil surgeon. The sealed envelope must be returned, unopened, to this office with any other requested evidence
3. The supplemental Form to I-693 submitted with your application to adjust status was not complete and is therefore unacceptable. According to the supplemental Form to I-693, you have not received the following required vaccination which have been recommended by the Centers of Disease Control and Prevention for someone of your age

Your original supplemental form to I-693 is hereby returned to you in the enclosed sealed envelope. Take this letter and your enclosed supplemental form to i-693 to the civil surgeon who administered your vaccinations. ONLY THE CIVIL SURGEON SHOULD BE OPEN THE SEALED ENVELOPE. The civil surgeon should administer any needed vaccinations and document your vaccination history on the supplemental form to i-693. If you cannot complete a vaccination serried because its not medically appropriate, the civil surgeon should request a waiver of that vaccination requirement on the supplemental form i-693. The civil surgeon should complete parts 2 and 3 of the supplemental form to i-693 and sign the form in the space provided
