Default I-94 got exchanged

My in-laws came to visit us on vistor visa(B1/B2) .
They got 6 month of stay in their I-94.
My father-in-law left US 2 day's before I -94 expiry but with my mother-in-law's I-94. Before he left I applied for the extension of stay(I539) for my Mother-in-law.
Her I-539 got denied stating the person has already left.
Now in my mother-in-law's passport we have my father-in-law's I-94. So actually at the port of entry the officer switched the I-94's.
Now the question is now how to handle the denial of extension for mother-in-law and get new I-94? and what to do with father-in-law's I-94.

I would suggest that take an info pass and meet an immigration officer and explain the situation to him personally and if possible given him a written explanation too. What i find a little wired that when you applied for extension you did not notice that that I 94s got exhanchged and even if you did you went ahead and applied for extansion. In my opinion never apply for extension as the applicants next visit is in jeopordy irrespective of result of extnsion of i 94 .

Thanks for your response. I did check the extension form many times and also reviewed it from another person. Because of their same last name and similar first name we all missed the mistake. We took an infopass appointment for tomorrow. Also we did go to CBP but they told us to file I 102 for my mother-in-law to get new I-94. Also I think I have to send the my father-in-law's I-94 to the following address for correction

Unfortunately now my mother-in-law has to go back early.
One thing I learned it to always check the full content of the I-94 after arrival.
