In the 485 check list sent by my employer one of the item is :

"Affidavit of support duly notarized (Form I-864) if you are applying for your family"

I am applying for my self and my wife and for my daugher. I will send my all paper to my employer and he will sent to attorney.
Do i have to fill form I-864 or attorney will fill form. if i need to fill then where i can find this form and how i can notarize it.

Please help.

is it for family based applications not for EB category fileing?

Use I-134 instead which is used for EB petitions.

InThemoment is right. I got I-134 from my attorney for 485 filing..

That is good information, thanks. Just one question:

- Was the I-134 part of your I-485 application, or was it part of your dependent's?

I am asking this because there is no instruction regarding such an affidavit in the Initial evidence section of the I-485.
