Default Green Card in Error

USCIS has approved divorced/ ex spouse green card. Who is responsbile for it ?
Will ex be able to use such a green card ? When it happens then does ex lose her non immigrant visa status ? does these non immigrant visa gets cancelled at the time of green card issuence ? Once green card is issued and if revoked to correct USCIS error then what will be the status of ex?

Did you notify USCIS when you divorced your then-wife? If so, then you have no other obligations.

Default Not an error!

Divorced does not mean that the spouse's green card application is cancelled! Moreover, the sponsorer (in this case the primary applicant) has already agreed to support the secondary applicant till they acquire 40 quarters of work experience when they originally supported the I-485 application. That is valid even after divorce. Any harassing actions such as falsely threatening to 'cancel' green card is illegal and punishable under US law!
