Default Documents required for h1b extension stamping

Hi Gurus:

I am planning to go for H1B extension stamping during first week of Nobvember 09. I am planning to take following documents. Please let me know whether I missed anything:

1. I797 approval notice for current employer and for all the previous employers.
2. Client letter.
3. All federal tax returns for the previous years.
4. All state tax returns for the previous years.
5. All W2s for the previous years.
6. Pay stub for past 6 months.
7. Bank statement.

I have following questions:

a. How many pay stubs I have to take? 3 months or 6 months or 10 months pay stub?
b. Do I need to get bank statement with balance confirmation or bank statement for 3 months or 6 months or 1 year?

I would greately appreciate your help on this.

Considering present mood of the agencies I would recommend a letter from current client or least vendor about your employment will be helpful. Applicable only if you are contracting not if you are fulltime at H1B sponsors location.
