Default Visitor visa application questions

Happy Holidays !. My parents (dad and step-mom) visitor visa to US is expiring next month which was sponsored by my sister 10 years ago. I am sending supporting documentation to my parents. I have few questions as below

1. Do i need to send I-134 for each applicant but I see that in I-134,step 3 asking for spouse name joining the applicant.
2. Do i need to mention as step-mother in the letter of invitation to my parents as my passport has my birth mother name (she died when I was a kid).
3. Do i have to give a copy of my birth certificate and passport to my parents

My parents 10 year visitor visa which was issued in Feb 2004 is expiring in Feb 2014. As they are not eligible for interview waiver, does it make any difference if they appointment before the visa expires (Feb 2014) or after ?
